- You as the Buyer understand that the use of any medical device can cause injury and even death if not used properly and in under specific condition. The Buyer also holds our company and its agents and affiliates harmless from any liability for the failure of this product which have been modified in any way from their original state nor for any failure due to misuse or application which is not in accordance with the designer's intention. Once payment is submitted, this will serve as purchaser agreement to releasing our company Surgik LC of any and all responsibilities of item such as stated above. By submitting payment you acknowledge that you have read and understood, and you agree, to the terms and conditions of this Agreement, and you represent that you are a AUTHORIZED LICENSED PURCHASER to enter this Agreement and become bound by its terms.
